Recently, research has shown that a 12-week program of pelvic floor muscle (PFM) training using extracorporeal biofeedback can be an effective and safe treatment option for female stress urinary incontinence without the discomfort caused by vaginal sensors. Long-term effects were investigated 12 months after treatment.

The strength of the PFM was significantly increased after the 12-week treatment. An objective cure (less than 2 g of urine leakage) was reached in 52% of the 106 subjects. Compliance was high and quality-of-life also significantly improved after treatment. Age and baseline pad weight were negative predictive factors for an objective cure of SUI.

Headache specialists have recognized a subgroup of migraine patients who remain refractory to treatment. Recently an overview of treatment approaches has been published. Combining various modalities can improve the chances for successful treatment.

The foundation of treatment is an emphasis on wellness. This includes optimizing mood, minimizing stress, practicing good sleep hygiene, and avoiding triggers. Biofeedback and cognitive behavioural therapy can be helpful in managing stress. All comorbid factors should be addressed, including chronic neck pain and obesity.

Preventive treatment is necessary in the majority of patients. Keeping a headache calendar is almost mandatory in management with attention to particular headache triggers, patterns and avoiding medication overuse.

The development of personalised medicine needs biomarkers to predict treatment outcome of specific treatments. Stratification of patient subgroups is one of the basic strategies in this approach. An open-label pilot study was performed in order to personalise the neurofeedback treatment of subgroups of patients with ADHD.

Pre- and post-treatment EEG and ERP changes were investigated in 21 patients that were treated at Brainclinics, Nijmegen. There was a significant improvement for inattention, hyperactivity / impulsivity and comorbid depressive complaints after QEEG-informed neurofeedback. This pilot is the first study that systematically demonstrated the effects of QEEG-informed neurofeedback in ADHD, however these results should be replicated in larger studies.
